pIOM® – Pelvic intraoperative neuromonitoring

Pelvic intraoperative neuromonitoring enables electrophysiological moni­toring of nerve functions during surgical interventions in the small pelvis. pIOM® has been proven to improve functional outcomes in patients under­going total mesorectal excision (TME), thereby contributing significantly to maintaining their quality of life.

pIOM® technology helps to preserve quality of life

Bladder voiding dysfunction, anal incontinence and sexual dysfunction are known complications of rectal or colon resections. In the small pelvis, the structures of the autonomic nervous system are complex and sometimes difficult to differentiate, which is why intraoperative neuromonitoring in this area is considered to be very important.

Pelvic intraoperative neuromonitoring is suitable for a variety of surgical interventions in the small pelvis and has so far been used primarily for the removal of rectal cancer in the course of deep anterior total mesorectal excision (TME). Pelvic intraoperative neuromonitoring can reduce the risk of complications. The pIOM® technology, patented in Germany, provides surgeons with significant support in maintaining the quality of life of many patients. This was demonstrated in 2022 in the study  “Pelvic intraoperative neuromonitoring prevents dysfunction in patients with rectal cancer: Results from a multicenter, randomized, controlled clinical trial of a NEURO­monito­ring System (NEUROS)” by Prof. W. Kneist et al.

pIOM® offers surgeons the possibility of an intraoperative nerve monitoring and thus also a better evaluation of the patient prognosis. It was honoured with the “Award for Patient Safety” by the German Society for Biomedical Engineering in the VDE and the German Coalition for Patient Safety.

C2 Xplore with special pIOM® software

The application of pelvic intraoperative neuromonitoring on the C2 Xplore and the pIOM® accessories is flexible, intuitive and user-friendly. With the help of the inoGuide, users are guided step by step through the procedure of pelvic intraoperative neuromonitoring with the pIOM® technology. With the help of the special pIOM® accessories, the anorectal function is continuously monitored during the measurement. Urinary function is monitored by measuring bladder pressure. Stimulation probes for open and minimally invasive laparoscopic and robot-assisted procedures are available for the stimulation of the auto­nomic nerves. If the nerves are successfully innervated, the surgeons are informed acoustically and visually about the activity. The simultaneous dis­play of EMG signals from the anal sphincter muscles and bladder pressure makes it easier for the surgeons to identify nerves and therefore to operate more gently. All events and comments are permanently stored on the C2 Xplore for documentation purposes and can be exported to a report for the patient record.
