Vascular surgery is dedicated to the treatment of various diseases of blood vessels. Intraoperative neuromonitoring plays an important role in this application field, especially during surgery of carotid artery stenosis caused by arteriosclerosis, for example.
Intraoperative neuromonitoring in vascular surgery
With the help of somatosensory evoked potentials (SEPs), incipient cerebral ischaemia can be indicated during vascular surgery. Electrophysiological early warning to protect against reduced blood flow in vascular surgery, and in particular during thromboendarterectomy on the carotid artery (carotid TEA), can be achieved by continuously recording SEPs (SEP monitoring). This method is a good alternative to Doppler sonography and helps surgeons to adapt their surgical strategy in good time in order to avoid possible postoperative neurological deficits.
Advances in technology are opening up further possibilities for advancing IONM. This means that surgical interventions in vascular surgery, such as thoracic or thoracoabdominal aortic aneurysm surgery, can be supported by IONM.

The specially developed 2-channel SEP software module for the C2 Xplore enables reliable stimulation of the somatosensory nerves and corresponding signal recording at the sensory cortex with a bilateral display of the SEP signals. This additional protection is specially designed for continuous monitoring against ischaemic events (bilateral SEPs).
In thoracoabdominal aortic surgery, spinal ischaemia also poses a risk, which can be detected by continuous SEP recording and additional motor evoked potentials (MEPs). The ISIS IOM system, which offers multimodal monitoring of the spinal cord pathways with additional channels and countless combination options, is particularly suitable for this purpose. Changes in SEPs and MEPs indicate malfunctions, providing increased risk protection for surgeons and patients during the live-saving procedure.
Overall, neurophysiological monitoring in vascular surgery, in particular the recording of SEPs, can improve the postoperative quality of life of patients.
